Clinical Philosophy — Whole-Person Care Guided by the 4Ms
At the Center for Healthy Aging, care is rooted in whole-person medicine and guided by the nationally recognized 4Ms Framework of Age-Friendly Care:
What Matters
Care starts with understanding what is most important to each patient personally — goals for independence, comfort, longevity, and quality of life — so all medical decisions align with those values.
Mentation
Attention to cognitive health, memory, mood, sleep, and emotional well-being because mental clarity and emotional health are essential to overall wellness.
Mobility
Protecting strength, balance, and independence through fall-risk assessment, safe movement guidance, and strategies to maintain physical activity.
Medications
Careful medication review and simplification to minimize side effects, interactions, and unnecessary prescriptions that often impact older adults and patients with chronic disease.
This framework allows Dr. Kohli to go beyond routine diagnosis-and-prescribing toward truly personalized primary care — treating the person, not just the problem.
His approach emphasizes:
- Deep listening and unhurried visits
- Clear education so patients understand their options
- Shared decision-making with patients and families
- Coordination across specialists and care settings
- A focus on safety, function, independence, and comfort
Why He Founded the Center for Healthy Aging
CFHA was built around one guiding question:
“What kind of clinic would I want for my own family?”
Dr. Kohli envisioned a practice where people would never feel rushed, unheard, or lost in the system — where patients receive both excellent medical expertise and genuine personal attention.
The result is the Center for Healthy Aging — a clinic intentionally designed around longer visits, continuity of relationships and true partnership with patients and families.
